Home Health Services
Ashland Community Hospital Home Health Services is a Joint Commission Accredited, Medicare Certified agency. It is the goal of the Home Health team to provide high quality, skilled health care to patients in their own homes. We care for people living in Ashland, Talent and the surrounding area and are available 24 hours a day, 7 days a week.
Services:
Skilled Nursing cares for those with illnesses such as cancer, diabetes, heart and lung disease, wounds, infections, and urinary incontinence, and instructs patients and their caregivers how to care for the illness. Physical Therapy treats problems such as polio, multiple sclerosis, Parkinson’s disease, stroke, and general debility following fractures and joint replacement surgery. Occupational Therapy evaluates and instructs patients in the use of adaptive equipment, the conduct of daily living activities, motor control, strengthening, and rehabilitation of perception and cognition. Speech Therapy evaluates and treats patients with difficulties swallowing, speaking or communicating following events like laryngectomy or stroke. Medical Social Work evaluates and assists patients with social, financial and emotional problems related to their illness.
Criteria for Admission:
• The patient must be homebound, meaning the patient is unable to leave their home without considerable effort.
• The patient must require skilled care from a registered nurse, physical therapist, or speech therapist.
• The patient must be under the care of a licensed physician.
• The patient and family/caregivers must consent to receive Home Health Services and be able to assist with the plan of care.
• The home environment must be safe and appropriate for the plan of care.
